Change in Repeat Prescription Order Process

Does a Pharmacy Order Your Prescription?  Please Read About the Changes in the Repeat Prescription Ordering Service The way repeat prescriptions are ordered is changing. From 1st April 2024 all patients (who are able to do so) should order their own medication directly from the GP practice – instead of your local pharmacy doing this for you.

Allied Health Professionals

Clinical Pharmacists help to ensure the quality of our prescribing.  They review medication changes following hospital discharge, resolve complex medication problems and help with the annual reviews we carry out for all patients on repeat medication.  Some clinical pharmacists are licensed to prescribe and may be involved in the management of long term health problems.

Our Clinical Pharmacist is Kirsty Conder, supported by Pharmacy Technician Helen Searle

Paramedics are highly experienced in the assessment and management of patients with urgent medical problems.  Working closely with our Urgent Care Team they carry out many of the home visits and also assess patients at Redlands Primary Care.

Occupational Therapist: Nikki

Community Midwives: Fiona, Tina, Sue, and Nikky, cover the Crediton area and can be contacted on 01363 771093.  They are based at Okehampton Hospital and also have a room at Crediton Hospital.

Community Nurses visit patients who are unable to get to the practice.  They are based at Crediton Hospital, and are led by the Community Matron Gae De La Fosse.

Health Visitors/Public Health Nursing Team: can be contacted on 0333 234 1902. Community Health Worker Gillian Gibson works alongside the health visitors.

Adult Mental Health Team: 01363 771100

Older Persons Mental Health Team serve those aged 65+ and are based at Crediton Hospital.

Wellbeing Service: a social prescribing project, which aims to help people who are lonely or isolated to improve their health and wellbeing by connecting them to community services and activities.
