The Practice will be closed from 1pm for training and development on the following dates: Telephone calls will be redirected to Practice Plus Group (PPG), patients requiring urgent medical attention that cannot wait until the practice reopens will be assisted by PPG. Register as a New Patient

Anyone in England can register with a GP surgery to access NHS services. It’s free to register.

Register online

We are using an NHS online service called Register with a GP surgery that makes it easy to register with this GP surgery.

Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.

The service is designed and run by the NHS, so your personal information is safe. It reduces our administrative workload and makes it easier for you to register.

Paper forms are still available if you need one.

OR download the forms below, complete both and hand into the practice.

New patient packs are also available for collection from both Crediton, and our Thorverton branch site.

Medical treatment is available from the date of registration. If you are on medication, you will need to speak to a doctor before a prescription can be issued.

Catchment area

We are happy to accept new patients who are ordinarily resident in the UK and living within our practice boundary.

You will need to complete a New Patient Registration Form, a New Patient Health Questionnaire and a Summary Care Record Preference Form, which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.

Please download and complete the three forms below for your registration to be processed. Once completed these can either be posted to us, emailed, or dropped into our post box outside the surgery.

Out of Area Registration

We are currently unable to accept “out of area” patient registrations. We need to ensure that we meet the needs of our current patients and prioritise those patients who live in our catchment area until such time as we have additional capacity. We apologise for any inconvenience.

To clarify, we are still accepting new patients who live within our catchment area; the only patients we are not currently accepting are those who live outside of our catchment area. To view our catchment area and check whether you live within it please use the catchment area tool above or speak to a member of staff. If you live within our catchment area and wish to register you will be most welcome.

Temporary Residents

If you will be living within our practice boundary for a maximum of 3 months, you may register as a temporary patient. Please contact your practice in the first instance and only contact us to register if they decide you need to be seen. If this happens, please call the practice on 01363 727111, or come into reception.

Organ Donation

From 2020, adults in England will be considered potential donors unless they choose to opt out or are excluded. For more information please visit the Organ Donation NHS UK where you can also register your decision.
