Redlands Patient Participation Group Application Form

Our Patient Participation Group (PPG) is a voluntary team of patients who work together with the practice to help improve services, strengthen communication, and ensure the patient voice is heard. By joining the PPG, you will have the opportunity to share your experiences, offer feedback, and contribute ideas that support the ongoing development of our practice. We welcome patients from all backgrounds, ages, and experiences. No specialist knowledge is required—just a willingness to share your perspectives and work collaboratively. Participation may involve attending occasional meetings, completing surveys, or taking part in discussions about how we can enhance the care we provide. Please complete the information below if you are interested in becoming a member of our PPG. Your involvement can make a meaningful difference to the quality of our services and the wellbeing of our community.
